Service Transition Plan

Service providers in the London-Middlesex, Oxford and Elgin areas wanted to implement a protocol for young people transitioning from youth to adult mental health and addiction services.They sought out mindyourmind to facilitate a design lab with a group of youth living in these communities. Together they created the Service Transition Plan, a guide to help youth, their families, and service providers prepare for the shift from youth to adult mental health and addiction services.

The hope is that this plan will allow for more successful transitions, and improved outcomes and experiences for youth. When you open the tool you will see that there is a version for the youth and another one for the service providers; the idea is that they will sit down together and work through the complementary guides while having their own copy that is relevant to them.
